Comment about Testicle Insertion: The testicles are saline-filled silicone prosthetics. We insert these in a second stage surgery, 6 months after the metoidioplasty. We prefer a staged approach because it decreases the biggest risk of testicle insertion which is infection of the implants or erosion of the implants through the skin. This is because when you insert the testicles during the original metoidioplasty surgery you are putting them inside a freshly created scrotum with fresh wounds. But in other centers they may do everything all at once to avoid a second trip to their center.
During the second stage surgery we also perform mons reduction. This means removing the small fat pad that is above the penis in the pubic hair area. This helps the penis "show" better. At the same time we can do any other "touch-up" surgery that is needed.
